CBY Youth Week 2020

ACKNOWLEDGEMENT AND WAIVER

The undersigned acknowledges that his or her child(ren) will be participating in the Congregation Bnai Yeshurun Youth Week at 641 West Englewood Avenue, Teaneck, NJ 07666 (the “Youth Week”) and has submitted or is submitting concurrently the registration form for the Youth Week.  The undersigned on behalf of himself, herself and family members further acknowledges and assumes all risks associated with their child’s or children’s participation in the Youth Week, including, without limitation, possible injuries that could occur while participating in the Youth Week.  The undersigned further acknowledges the continued presence of the COVID-19 pandemic (the “Pandemic”) and assumes all risk associated with the Pandemic, including, without limitation, the possible dissemination of the Pandemic from other children participating in the Youth Week, from group leaders of the Youth Week and from other parents.  The undersigned hereby releases Congregation Bnai Yeshurun and its officers, directors, members and employees from and against any and all claims, liabilities, actions and damages that the undersigned or any of the undersigned family members may sustain as a result of the undersigned’s child’s or children’s participation in the Youth Week.
