ACKNOWLEDGEMENT AND WAIVER

            The undersigned acknowledges that he/she will be attending programming at Congregation Bnai Yeshurun, 641 West Englewood Avenue, Teaneck, New Jersey 07666 ("Programming”) in accordance with the registration form prepared and distributed by Congregation Bnai Yeshurun.  The undersigned further acknowledges the continued presence of the COVID-19 pandemic (the “Pandemic”) and assumes all risk associated with the Pandemic, including, without limitation, the possible dissemination of the Pandemic from other attendees of the Programming.  The undersigned hereby releases Congregation Bnai Yeshurun and its officers, directors, members and employees from and against any and all claims, liabilities, actions and damages that the undersigned or any of the undersigned family members may sustain as a result of the undersigned’s attendance at the Programming.
