The undersigned acknowledges that both he/she and his or her child(ren) will be participating in the Congregation Bnai Yeshurun David Frohlich z"l Youth Department Winter Break 2021 Activities ("DFYD Winter Break") at 641 West Englewood Avenue, Teaneck, NJ 07666 and at Campgaw,  and has submitted or is submitting concurrently the registration form for DFYD Winter Break.  The undersigned on behalf of himself, herself and family members further acknowledges and assumes all risks associated with their child’s or children’s participation in the DFYD Winter Break, including, without limitation, possible injuries that could occur while participating in the DFYD Winter Break.  The undersigned further acknowledges the continued presence of the COVID-19 pandemic (the “Pandemic”) and assumes all risk associated with the Pandemic, including, without limitation, the possible dissemination of the Pandemic from other children participating in the DFYD Winter Break and from other parents.  The undersigned hereby releases Congregation Bnai Yeshurun and its officers, directors, members and employees from and against any and all claims, liabilities, actions and damages that the undersigned or any of the undersigned family members may sustain as a result of the undersigned’s child’s or children’s participation in the DFYD Winter Break.
